Find a Provider

In order to maximize the benefits under your medical plan, it’s important to select a provider within your assigned network. Look at your 3PA ID card for the name of the appropriate network and then select from the choices at the right to perform a search for physicians, clinics and facilities in your network. Most plans we administer do not require a referral when seeing a specialist but do call to double check before making an appointment. If you have Dental and Vision Coverage with us it is not necessary to select a provider from these lists, you may choose your dental and vision provider.
